Physical Examinations
Grade and sports physicals will conducted at the Jr-Sr High School by the school physician on Wednesday, May 28th, 2008.  School physicals will not be offered again until October, 2008.  Any student entering the district this year, as well as all 7th and 10th graders must have a completed current physical form on file in the health office (dated within 12 months of the first day of classes) within 30 days of the beginning of the 2008-2009 school year.  Students wishing to participate in sports must also have a current physical on file (within 12 months of the first day of tryouts for the sport season)   Ms. Rohe must qualify each athlete before they can participate in the sports program; qualification requires a review of the completed physical form (required annually) and the health history form (completed by the parent within 30 days of the first day of tryouts for EACH season).  The qualification process does take some time; therefore parents need to be sure all athlete's forms are dropped off/mailed to Ms. Rohe prior to tryouts.  It is suggested that these examinations be done by your family physician, as he/she is most familiar with your child and is better able to judge any change or problem in your child's health status.  Your primary care provider may need to update your child's immunizations as well.  Summer is a good time to schedule these appointments.  However, school physicals are provided each year for those parents who would prefer to have their child's exam done at school.  The May 28th school physical will cover your student for the fall sports season as well as the required school physical for all 7th and 10th graders. Medications

New medication forms for the 2008-2009 school year will be sent home to all students who were on medications this year.  As you are aware, we need a new physician's order and parental permission each school year.  Please have your child's physician complete the Parent and Prescriber's Authorization for Administration of Medication in School form if your son/daughter is to receive any medications this school year. If your child is to carry and self-administer their medication (such as an inhaler), please have the Self-Medication Release Form completed by doctor and parent as well.  These forms can be printed from this website.
