As of September 29, 2005, there are new guidelines from The State Education Department regarding the required health screenings in New York State schools. Below is a list of the changes that will be implemented for the Onondaga Central School District:Vision: Screening and recording of the following visual tests:*All new entrants to school, within six month of admission, a minimum of color perception, distance acuity, near vision and hyperopia.*Screening and recording of distance acuity for grades K,1,2,3,5,7 and 10*At any time deemed necessary by school authorities.Hearing: Screening and recording of the following:*Hearing screening must be administered to all new entrants within six months of admission to the school.*Screening and recording of hearing for grades K,1,3,5,7 and 10*At any time deemed necessary by school authorities.Scoliosis: Screening and recording of the following:*Scoliosis screening must be administered once each school year for all students in grades 5 through 9.If you have any concerns regarding your child's vision, hearing or spinal alignment, the school nurse will be happy to screen your child regardless of grade level.Below is a summary of guidelines regarding school health appraisals.Physical examinations required when children:
* Enter the school district for the first time at any grade level, entering Pre-K or kindergarten* Are in grades 2, 4, 7 and 10* Participate in interscholastic sports* Need working papers* Are referred by/to the Committee on Special Education* Are deemed necessary by school authorities to determine a child's education program.Physicals performed within the last 12 months may be accepted.Physical examinations are offered by the school physician twice during the school year; usually in October and March. Physical examinations may also be submitted from your primary health care provider.Sports physicals are valid for a 12-month period through the last day of the month in which the physical was conducted. If the 12-month period for the physical expires during a sports season, participants may complete the season. Sports season includes tryouts. Physical Education Excuses: If a student needs to be excused from physical education classes, parents are required to send a written excuse to the school nurse. If your child must be excused for more than one week, a note from the attending physician is required. |

Medication Procedure: In order for a student to receive medication in school the "Parent and Prescriber's Authorization for Administration of Medication in School" form must to completed by both the parent and the prescribing physician. This pertains to both over-the-counter and prescription drugs. A new form must be completed each school year. These forms may be obtained from the school nurse or can be printed from the OCS website. All medications must be in a properly labeled original pharmacy container. Over the counter medications must be in the original manufacturer's container/package with the student's name on the container. Medications are kept in the health office and are stored in a locked cabinet. Students are not allowed to carry any medication in school (*See exception below). This includes all prescription and over-the-counter medications. *Under certain conditions a student may carry and self-administer his/her own medication (i.e. an inhaler for asthma). If this is a consideration, contact the school nurse for information. Additional documentation is necessary from the physician and the parent in this instance. Students are not allowed to carry or self-administer medication without proper authorization. Self-Medication Release Forms can be printed from this website.
